Important laws and ethical principles contribute to a safe and therapeutic experience in counseling. Reading and understanding my policies is essential to knowing your rights, protections, risks, and responsibilities. Please seek to discuss with me any questions or concerns you have about my policies. The Federal Health Insurance Portability and Accountability Act (HIPAA) requires mental health professionals to issue this official Notice of Privacy Practices. This notice describes how information about you is protected, the circumstances under which it may be used or disclosed and how you may gain access to this information. Please review it carefully.

There are times when a person other than the client is paying for my services. This form discusses important items related to fees, confidentiality, and the termination of the agreement. It also serves as the contract between the person paying for services and Athens Professional Counseling Services, LLC. The person paying for services completes and returns this form to Jason.
